Applying for registration is now handled on-line through the NHS Register with a GP surgery service:

The online process is quick and easy to complete and you will be guided through a series of questions to help you complete the required details.

You can also apply from the NHS App.

  • Once you have completed your application we will verify your details and process your registration.

  • If we have any issues with your registration you will be contacted using the supplied details.

  • We may require additional evidence or information in order to register you.

  • PMG reserves the right to refuse registration as set out in our patient registration policy.

  • We aim to process all new applications within 7 working days.

  • You will receive confirmation of your registration by post or e-mail once complete.
     

Applying online is the quickest and most accurate way to get registered. However, if you would rather use a paper form, this may be downloaded here for printing or a copy requested in practice. Our staff are also on hand in practice to help you with completing online registration.
